Suspend Contract Form

You may suspend your Automatic Billing for a maximum of one (1) billing cycle per twelve (12) month period if a minimum of seven (7) days notice is given in writing via the  form. You must provide written notice using the Suspend/Cancel form at least seven (7) days in advance of your scheduled payment cycle (Automatic Billing date) which can be found on your application. If the suspension is not submitted at least seven (7) days before your next scheduled Automatic Billing date, you will be responsible for payment. A verbal communication, email, or text will not be considered official communication of intent to suspend. This is the only form that may be used as intent to suspend a recurring contract.
