PAYMENT/FEES (RECURRING LESSONS): To take advantage of the discounted monthly pricing, you must complete the application for recurring lessons. Monthly lesson fees will be automatically charged to the card on file on the payment date chosen in your application. You will be allowed to choose your preferred payment date on your application. If your first lesson(s) start before or after your chosen automatic billing date, your first billing will be prorated. Automatic Billing usually occurs between 1:00 am and 8:00 am CST on the chosen payment day you selected in your contract. A $75.00 late fee will be assessed by 12:00 pm on your chosen payment day if payment cannot be collected on the payment day selected in your contract. An additional daily $10.00 late fee will be assessed for each late day after, until payment in full is collected. Your contract may also be subject to termination if lesson payment and any assessed fees remain unpaid. We do not accept cash or any other form of payment on site.
CANCELLATION/RESCHEDULING/ABSENTEEISM: You may cancel or reschedule your lesson via the Acuity Scheduling widget on our website (in the SCHEDULE menu) or the Acuity Scheduling App (available in the Apple or Google Store) up to 10 minutes before your scheduled lesson. Canceled lessons are forfeited and will not be credited, refunded, or rescheduled. Therefore, we suggest that you reschedule your lesson. You are allowed to reschedule your lesson only one (1) time. If you need to reschedule your lesson a second time, please contact the vocal studio for prior approval. Lessons that are rescheduled a second time without approval will be automatically deleted by the system. If you fail to attend your lesson twice without officially canceling or rescheduling via the Acuity Scheduling widget on our website or the Acuity App as described above, those lessons are forfeited and cannot be credited, refunded, or rescheduled.
CLOSED DAYS/INSTRUCTOR CANCELLATION/HOLIDAYS/VACATION: We are closed the 29th to 31st of every month. Those days fall on the fifth (5th) week of the month; therefore, you are not charged for that day since we only charge for four (4) lessons per month. If your instructor must cancel a lesson, your upcoming billing will be automatically adjusted/credited. Lessons will not be refunded. If the studio is closed for a holiday spanning more than one day, your billing will be adjusted automatically for each lesson that is affected. Instructor may choose to work during vacation time. If, in the event the instructor chooses not to work, your billing will be automatically adjusted as described above.
SUSPEND AUTOMATIC BILLING: You may suspend your Automatic Billing for a maximum of one (1) billing cycle per twelve (12) month period. You must provide notice using the Suspend Contract form at least seven (7) days in advance of your scheduled payment cycle (Automatic Billing date) which can be found on your application. If the suspension is not submitted at least seven (7) days before your next scheduled Automatic Billing date, you will be responsible for payment. Verbal, emailed, or texted requests for suspension or cancellation are not considered official.
CANCEL CONTRACT: You may cancel your contract with thirty (30) days’ notice. You must provide written notice of your intent to cancel your contract via the Cancel Contract form at least thirty (30) days in advance of your scheduled payment cycle (Automatic Billing date) which can be found on your contract. If the contract cancellation is not submitted at least thirty (30) days before your next scheduled Automatic Billing date, you will be responsible for payment. Verbal, emailed, or texted requests for suspension or cancellation will not be considered official.
IN-PERSON AND ONLINE LESSONS: Enrolled clients may choose to have in-person and online lessons for their convenience. The Damon K Clark Vocal Studio reserves the right, temporarily or permanently, to move any and/or all client lessons online at any time.
CHANGES/UPDATES/CLARIFICATIONS TO TERMS AND CONDITIONS: The Damon K Clark Vocal Studio reserves the right to make changes, updates, and/or clarifications to these terms and conditions at any time. Clients/Students will be notified of any changes to these terms and conditions in writing via the client/student email address provided in their contract.
ACKNOWLEDGEMENT: By Submitting application to the Damon K Clark Vocal Studio, you fully acknowledge that you have read the Terms and Conditions and authorize automatic payment to the Damon K Clark Vocal Studio as per the Terms and Conditions.
