PAYMENT/FEES (RECURRING LESSONS): To take advantage of the discounted monthly pricing, you must complete the application for recurring lessons. Monthly lesson fees will be automatically charged to the card on file. You will be allowed to choose your preferred payment date on your application. If you have a lesson or lessons scheduled prior to your first automatic billing date, the card on file will be charged for the lessons. Automatic Billing usually occurs between 1:00 am and 10:00 am CST on the chosen payment day you selected in your contract. A $50.00 late fee will be assessed by 12:00 pm on your chosen payment day if payment cannot be collected on the payment day selected in your contract. A additional daily $5.00 late fee will be assessed for each late day after until payment in full is collected. Your contract may also be subject to termination if lesson payment and any assessed fees remain unpaid. We do not accept cash or any other form of payment on site.
CANCELLATION/RESCHEDULING/ABSENTEEISM: You may cancel or reschedule your lesson via the Acuity Scheduling widget on our website (in the SCHEDULE menu) up to 10 minutes before your scheduled lesson. Canceled lessons are forfeited and will not be credited, refunded, or rescheduled. Therefore, we suggest that you to reschedule your lesson if at all possible. If you fail to attend your lesson twice without canceling or rescheduling as described above, those lessons are forfeit and cannot be credited, refunded, or rescheduled.
CLOSED DAYS/INSTRUCTOR CANCELLATION/HOLIDAYS/VACATION: We are closed the 29th to 31st of every month. Those days fall on a fifth (5th) week of the month, therefore you are not charged for that day since we only charge for four (4) lessons per month. If your instructor must cancel a lesson, your upcoming billing will be automatically adjusted/credited. Lessons will not be refunded. If the studio is closed for a holiday spanning more than one day, your billing will be adjusted automatically for each lesson that is affected. Mr. Clark may work during vacation time. If, in the event he chooses not to work, your billing will be automatically adjusted/credited.
SUSPEND AUTOMATIC BILLING: You may suspend your Automatic Billing for a maximum of one (1) billing cycle per twelve (12) month period if a minimum of seven (7) days notice is given in writing via the Suspend/Cancel form. You must provide written notice using the Suspend/Cancel form at least seven (7) days in advance of your scheduled payment cycle (Automatic Billing date) which can be found on your application. If the suspension is not submitted at least seven (7) days before your next scheduled Automatic Billing date, you will be responsible for payment. A verbal, emailed, or texted requests for suspension or cancellation will not be considered official. Note: As an alternative, you may opt to take Single Lessons instead of suspending/canceling your contract by submitting a new application for Single Lessons at full lesson price. The new application must be submitted a minimum of seven (7) days ahead of your Automatic Billing date. This will automatically deactivate your Automatic Billing setup and you will remain a student.
CANCEL CONTRACT: You may cancel your contract with thirty (30) days notice. You must provide written notice of your intent to cancel your contract via the Suspend/Cancel form at least thirty (30) days in advance of your scheduled payment cycle (Automatic Billing date) which can be found on your contract. If the contract cancellation is not submitted at least thirty (30) days before your next scheduled Automatic Billing date, you will be responsible for payment.
IN-PERSON AND ONLINE LESSONS: Enrolled clients may choose to have in-person and online lessons for their convenience. The Damon K Clark Vocal Studio reserves the right to move to temporarily move any client's lessons online at any time.
CHANGES/UPDATES/CLARIFICATIONS TO TERMS AND CONDITIONS: The Damon K Clark Vocal Studio reserves the right to make changes, updates, and/or clarifications to these terms and conditions at any time. Clients/Students will be notified of any changes to these terms and conditions in writing via the email address provided in their contract.
ACKNOWLEDGEMENT: By Submitting application to the Damon K Clark Vocal Studio, you fully acknowledge that you have read the Terms and Conditions and authorized Automatic Payment to the Damon K Clark Vocal Studio as per the Terms and Conditions.
