Cancel Contract Form

You must provide written notice of your intent to cancel your contract via the Cancel Contract Form at least thirty (30) days in advance of your scheduled payment cycle (Automatic Billing date). If the contract cancellation is not submitted at least thirty (30) days before your next scheduled Automatic Billing date, you will be responsible for payment. A verbal communication, email, or text will not be considered official communication of intent to cancel. This is the only form that may be used as intent to cancel a recurring contract.
